{"id":479,"date":"2023-07-10T13:50:57","date_gmt":"2023-07-10T13:50:57","guid":{"rendered":"https:\/\/mucenterofexcellence.org\/?page_id=479"},"modified":"2023-10-25T16:44:36","modified_gmt":"2023-10-25T16:44:36","slug":"west-virginia-intensive-clinical-care-coordination-team-wviccc","status":"publish","type":"page","link":"https:\/\/mucenterofexcellence.org\/projects\/west-virginia-intensive-clinical-care-coordination-team-wviccc\/","title":{"rendered":"West Virginia Intensive Clinical Care Coordination Team (WVICCC)"},"content":{"rendered":"\n<div class=\"wp-block-uagb-container uagb-block-5fe72d2c alignfull uagb-is-root-container\"><div class=\"uagb-container-inner-blocks-wrap\">\n<div class=\"wp-block-group alignwide is-layout-constrained wp-block-group-is-layout-constrained\">\n<p class=\"has-text-color\" style=\"color:#046a38;margin-bottom:0;padding-bottom:0\">PROJECTS<\/p>\n\n\n<h2 style=\"padding-top:0;margin-top:0;\" class=\"has-text-align-left alignwide wp-block-post-title\">West Virginia Intensive Clinical Care Coordination Team (WVICCC)<\/h2><\/div>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-columns alignwide is-layout-flex wp-container-core-columns-is-layout-1 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<p>This team works with the Bureau of Social Services to provide assessments, discharge planning, and implementation for youth in the custody of the state that are placed in out-of-state residential treatment facilities. <strong>The goal of the team is to bring youth back to West Virginia into community level services that are effective in keeping the youth at home and not returning to a higher level of care.<\/strong> This team will work with the out-of-state provider, DHHR case worker, and community partners to accomplish this goal.<\/p>\n\n\n\n<ul>\n<li><strong>Assessments<em> <\/em><\/strong>\u2013 The team utilizes the&nbsp;Transformational Collaborative Outcomes Management tool (TCOM), Child Adolescent Needs and Strengths tool (CANS), Child and Adolescent Functional Assessment Scale (CAFAS), and the Preschool and Early Childhood Functional Scale (PECFAS).
