West Virginia Intensive Clinical Care Coordination Team (WVICCC)

This team works with the Bureau of Social Services to provide assessments, discharge planning, and implementation for youth in the custody of the state that are placed in out-of-state residential treatment facilities. The goal of the team is to bring youth back to West Virginia into community level services that are effective in keeping the youth at home and not returning to a higher level of care. This team will work with the out-of-state provider, DHHR case worker, and community partners to accomplish this goal.

  • Assessments – The team utilizes the Transformational Collaborative Outcomes Management tool (TCOM), Child Adolescent Needs and Strengths tool (CANS), Child and Adolescent Functional Assessment Scale (CAFAS), and the Preschool and Early Childhood Functional Scale (PECFAS). The team will also use the WV Decision Support model that is based on the CANS tool.
  • Implementation – The team will collaborate with the system of care and based on the assessments completed, will make appropriate referrals to link families and children with after care services.
  • Agency Partners – Bureau for Social Services, AETNA, Children Serving Agencies
